📋 Tanggung Jawab Pekerjaan

Provide pharmaceutical care to patients, including medication therapy management and disease state management.,Ensure the safe and effective use of medications by patients, including monitoring for adverse drug reactions and drug interactions.,Administer vaccinations and other immunizations as needed.,Collaborate with healthcare providers to ensure the optimal use of medications and other therapies.,Maintain and update patient medication profiles and medication administration records.,Participate in quality improvement activities and clinical research as needed.,Perform other duties as assigned by the supervisor.

🎓 Kualifikasi & Syarat

Bachelor's degree in Pharmacy from an accredited institution.,Licensed pharmacist with a valid pharmacy license.,Minimum of 2 years of experience in a clinical pharmacy setting.,Strong knowledge of pharmaceutical care and medication therapy management.,Excellent communication and interpersonal skills.,Ability to work well in a team and independently.,Familiarity with electronic health records and pharmacy information systems.,Ability to multitask and manage time effectively.
